Esowen Injection

Esowen Injection is a prescription medicine containing Esomeprazole 40 mg, a proton pump inhibitor (PPI). It is used in clinical and hospital settings for the short-term management of acid-related gastrointestinal disorders, particularly when oral therapy is not suitable, such as in patients who are nil-by-mouth, post-operative or with severe symptoms.

Esowen Injection is a lyophilized powder for injection supplied with a sterile diluent. Each vial contains:

It is intended for intravenous (IV) or intramuscular (IM) use under medical supervision and is commonly used as a short-term bridge to oral PPI therapy.

Esomeprazole, the active ingredient in Esowen Injection, works by selectively and irreversibly blocking the H+/K+ ATPase (proton pump) in the gastric parietal cells. This is the final common pathway for gastric acid secretion.

A doctor may prescribe Esowen Injection for:

• Gastroesophageal reflux disease (GERD) and erosive oesophagitis
• Peptic ulcer disease, including gastric and duodenal ulcers
• Prevention and treatment of NSAID-associated ulcers
• Upper gastrointestinal bleeding due to peptic ulcer (as part of combined therapy)
• Zollinger-Ellison syndrome and other pathological hypersecretory conditions
• Acid-related conditions in patients unable to take oral medication

Esowen Injection is used only when clinically indicated and under medical supervision.

The dose and duration of Esowen Injection are decided by the treating physician based on the indication, severity of the condition and overall clinical status of the patient.

• Administered as a slow IV injection (over at least 3 minutes), IV infusion (10–30 minutes) or as IM injection
• Typically 40 mg once daily; higher doses may be used in acute bleeding or hypersecretory states
• Must be reconstituted with the supplied diluent and used immediately after preparation
• Treatment is usually continued until oral therapy can be resumed

Dose adjustments may be required in patients with severe liver impairment.

Like all medicines, Esowen Injection may cause side effects, although not everyone experiences them.

Common side effects:

• Headache
• Nausea or vomiting
• Abdominal pain or flatulence
• Diarrhoea or constipation
• Dry mouth
• Mild reaction at the injection site

Less common or serious side effects:

• Allergic reactions including skin rash, itching or swelling
• Dizziness or sleep disturbances
• Low magnesium, sodium or vitamin B12 levels with prolonged use
• Liver enzyme elevations
• Bone fractures with long-term high-dose PPI use

Severe or persistent symptoms should be reported to the medical team immediately.

Before receiving Esowen Injection, the doctor should be informed if you:

• Have a known allergy to esomeprazole, omeprazole or other PPIs
• Have a history of severe liver disease
• Have low magnesium levels, osteoporosis or risk factors for fractures
• Have warning symptoms such as unintentional weight loss, persistent vomiting, difficulty swallowing or vomiting blood
• Are pregnant, planning a pregnancy or breastfeeding
• Are taking other prescription or over-the-counter medicines, including herbal products

Long-term use should be reviewed periodically by the prescribing doctor.

Esowen Injection may interact with certain medicines, including:

• Clopidogrel and other antiplatelet agents
• Warfarin and other oral anticoagulants
• Antiretroviral medicines used for HIV
• Methotrexate (especially at high doses)
• Digoxin, ketoconazole and itraconazole
• Other acid-suppressing medicines

Always share a complete list of medications, supplements and herbal products with the healthcare team.

• Store the unopened vial in a cool, dry and dark place below 25°C
• Protect from direct light and moisture
• Use immediately after reconstitution; do not store the prepared solution
• Keep out of the reach of children
• Discard unused portion as per institutional protocol
